Referral Program Terms & Conditions

The 4Ever Young Referral Program rewards both existing patients (“Referrer”) and new patients (“Referee”) with a $100 credit when eligibility requirements are met. 

Eligibility & Redemption: 

• The Referee (new patient) will receive a $100 credit applied to their first invoice upon their initial visit, provided a minimum purchase of $200 is met. 
• The Referrer (existing patient) will receive a $100 credit once the Referee completes their first qualifying visit and transaction. 

Referral Code Requirement: 

• The Referrer’s unique referral code must be added to the Referee’s profile prior to closing the Referee’s first invoice. 
• Failure to apply the referral code at the time of the first visit will result in forfeiture of referral benefits for both parties. 

Exclusions: 

• Referral credits are not valid toward the purchase of gift cards, prepaid cards, or memberships. 

Stacking & Usage: 

• Referral credits may not be combined with membership credits or any other promotions, discounts, or offers. 
• Credits have no cash value and are non-transferable. 

Program Limits: 

• Only one referral promotion may be active at a time. 4Ever Young reserves the right to modify or replace the referral offer at any time. 

Additional Terms: 

• Referral credits are issued as account credits and must be used toward eligible services or products. 
• 4Ever Young reserves the right to verify eligibility and refuse or revoke credits in cases of suspected misuse or abuse of the program. 
• Program terms are subject to change at any time without prior notice.
